Transferring Gas Connection

Jotting down the process while transferring my connection from Mangalore to Bangalore.
I had Bharat Gas Connection

The documents which are required:
1. Voucher (which the earlier Gas Agency will give once you return your cylinder and regulator).
2. Address Proof copy (rent agreement copy)
3. Electricity Bill copy
4. Id Proff copy
5. Affidavit

This Affidavit took me time and so thought of putting the process.

The Affidavit is required on the Rs 2 stamp papaer (that's it). Nothing else is required. You can go to any of the Sub-Registrar office and there will be photocopy/printout shops who will have these stamp paper. They also have the Gas Connection Affidavit format. This costed me 35 bucks. What extra I did (and which is not required) is that I went inside office and got it registered (they put some 20 Rs for this). This is not required.
If you can get hold of Rs2 stamp paper, you yourself can get the printout done - getting stamp paper though is problematic.
Some Notary people will ask for 100 Rs or so, don't get that done from there as all those things are not required at all. Just the printout on the required Rs 2 stamp paper is what is all needed.
